A Clinical Review of Upper Gastrointestinal Tract Bleeding

Abstract
A total of 330 patients of upper gastointestinal tract bleeding, who were admitted in Yonsei
University Hospital during the past ten years from Jan. 1961 to Dec. 1970, were clinically reviewed
The following results were obtained.
1) The incidence of upper gastointestinal tract bleeding was 0.36% of all patients admitted to Yonsei
University Hospital.
2) The causative disease producing upper gastointestinal tract bleeding were peptic ulcer(49.3%),
esophageal varices(20.6%), stomach Ca.(8.5%), hemorrhagic gastrics(6.1%), drug induced acute
ulcer(5.2%), stress ulcer(1.5%), miscellaneous(2.4%) and undetermined(6.4%).
3) The sex and age incidence of gastointestinal tract bleeding revealed a greater prevalence in male and
4th decade, with a ratio of 5.5 males to 1 females.
4) On admission, hematemesis was noted in 279 patients(72.4%), melena in 170 patients(51.5%)
and combined hematemesis and melena in 111 patients(33.6%), 124 patients(37.5%) had massive
upper gastointestinal tract bleeding with lower Hb. level(below the 7gm%) and 79 patients(24%)
was in shock state.
5) 5 patients(1.5%) had massive transfusion over 30 pints of whole blood, 9 patients(2.7%) 11¡­30
pints, 3 patiens(9.7%) 11¡­20 pints and most of patient(86%) 1¡­10 pints.
6) Among 330 cases, 108 cases(32.7%) were operated and 222 cases(67.3%) were not operated.
7) Among operated cases, subtotal gastrectomy was most common procedure.
8) Post op. complications were 18 cases (16.6%) which were wound infection, wound disruption,
atelectasis, pneunomina, bleeding, fistula and hepatic coma.
9) The over all mortality rate was 8.8%(29 cases) and the mortality rate for patients receiving surgical
treatment was 5.5% (6 cases), while for the conservatives were 10.3% (23 cases). The cause of
death were mainly due to sustaoned bleeding and shock, and hepatic failure, etc.
